Special driving licences: how they work and what adaptations they require

What a special driving licence means, who assesses fitness to drive, and where to find prescribed codes and adaptations.

A special driving licence certifies fitness to drive subject to requirements or adaptations set after medical and technical assessment. Do not self-diagnose: the Local Medical Commission is the point of reference.

Individual fitness assessments and requirements

A special licence is issued to people whose fitness to drive requires a specific assessment and, in some cases, vehicle adaptations or restrictions. A diagnosis alone does not determine the outcome: the relevant authorities assess each person individually, usually through the Local Medical Commission. The Commission may request further examinations and set requirements, while the Motor Vehicle Office handles the relevant administrative and technical matters.

Authorised conditions are also recorded as harmonised codes on the licence. A code may identify an adaptation, restriction or condition of use. Do not interpret it alone if the number is incomplete or you do not have the latest official guide: contact the issuing office or Commission. Do not modify the vehicle or drive a different one until you have checked that it complies.

  • The assessment is individual and involves medical and administrative authorities.
  • Codes on the document set out requirements you must follow.
  • Adaptations must match the licence and vehicle documents.

Preparing for the assessment

Check the Motorist’s Portal or your local health authority (ASL) for the Local Medical Commission procedure, required documents and booking arrangements. Requirements may depend on your case: bring up-to-date medical certificates and requested documents, rather than relying on generic lists that may not apply locally. If you already hold a licence and your health changes, promptly ask the relevant authority what to do; do not suspend or change your requirements yourself.

When an adaptation is prescribed, compare the licence requirements with the vehicle and ask a qualified vehicle modifier, insurer and Motor Vehicle Office for confirmation within their respective areas. A code is not general approval for every technical solution. Maintaining the adaptation is also part of everyday safety.

Theory study

These requirements also have a practical purpose: they help identify a suitable vehicle in advance and reduce the risk of incompatibility. When booking a hire car or requesting a replacement vehicle, explain the restriction without sharing more health information than necessary and ask for written confirmation of the available configuration. If an adaptation fails or the vehicle breaks down, do not improvise another solution: contact the installation centre and relevant authority for guidance.

For theory revision,Quiz Patente Ufficiale lets you study by chapter, practise with timed quizzes and review mistakes, without replacing required assessments.
